At Hollies Surgery, we work closely with our Primary Care Network (PCN) to provide access to Social Prescribing Link Workers.
Our Social Prescribing team can support you with a wide range of non-medical needs that may be affecting your health and wellbeing, such as:
- Social isolation and loneliness
- Mental wellbeing and emotional support
- Financial or housing concerns
- Access to community groups and local services
- Lifestyle and wellbeing support
You may be referred to one of our Social Prescribing Link Workers, or you can ask a member of our team for more information.

Care Coordination Support
At Hollies Surgery, we work with our Primary Care Network (PCN) to provide access to Care Coordinators, who help patients navigate services and manage their care more effectively.
Care Coordinators can support you with:
- Coordinating appointments and follow-ups
- Helping you understand your care plan
- Linking you with appropriate services
- Supporting patients with complex or ongoing health needs
- Improving communication between services
